By submitting an application, you are certifying that all details supplied in the application and in any attached documents are true and correct to the best of your knowledge, and that the application has been submitted with the full knowledge of the submitter, their family or guardian.

I understand that the information above will be used in accordance with relevant legislation and declare that this information is correct to the best of my knowledge.

I also agree that a final acquittal report that includes financial statement and a narrative of the activities undertaken and certified by a Disability Programs Manager of the organisation will be required. If you have a pre-existing format for the acquittal report, a sample can be provided with the application for consideration. Alternatively, Hartley can provide a sample acquittal report. 

A final grant acquittal report as outlined above will be due within three months of project completion unless developments of the pandemic require otherwise.  

Grantees may receive a site visit, telephone call and/or other type of communication from Hartley Management, requesting updates as part of compliance monitoring processes.

Hartley may periodically request impact stories and/or photographs of project progress or completion for possible inclusion on its website, social media outlets or annual report (photographs that feature faces require permission from all individuals or their guardians).

Established in 1962, Hartley Lifecare (Hartley) is a Canberra-based not-for-profit organisation providing supported accommodation for people with disability, their families and carers. Originally established to provide a learning facility and therapy support service for children and adults, Hartley has grown extensively to support clients in over 30 homes across the ACT. Over the last six decades, families have grown to rely on and trust Hartley to deliver high quality, person centred support.
